Intervention and Evaluation Plan

Problem Statement

The client reported an atypical mental/cognitive, social, and psychological development during her childhood. This happened due to all the sexual and emotional abuse that occurred from a very young age by her parents. Shows signs of displacement when she takes her anger on other people and she explained that is why sometimes she responds with an aggressive tone. Due to all these events that occurred in her life, she has trouble communicating and expressing herself with other people. She believes that if she doesn’t work in this area (anger management) it will be difficult for her to get her first job, which is very important since she needs to get a job to be able to find a stable place and support her children.

Goal Statement

Client will be able to manage anger by effectively expressing her needs and feelings through communication in social settings within three months.

Objectives/Tasks

1. The client will be instructed by the social worker to go to her first therapy session within 7 days and will report back to the social worker to start working on her anger management and communication problems by Monday 11/07/23.

a. the social worker will provide a list to the client of local therapists in her area.

b. the social worker will assist the client with a transportation pass to and from the therapy appointment.

c. Next appointment the client will bring a list of current communication problems that she would like to overcome in order to get better.

2. Client will be able to identify her communication triggers so she can learn skills to communicate when angry in weekly meetings by 01/02/23 .

a. The client will gather a list of her social anxieties that upset her.

b. The social worker will gather helpful management strategies for the client to apply when in stressful situations.

c. The client will work alongside the social worker to visit social settings at least twice a week to identify when she may be feeling triggered.

3. Client will share 3 positive actions in which she feels emotionally accomplished during each week by Monday 01/02/22.

a. Client will record in a journal her feelings and accomplishments she faced during the week.

b. The client will work alongside the social worker to review her positive experiences and role-play them for communication improvements daily.

c. The client and social worker will assess her needs and feelings during each positive experience discussed so the client can acknowledge them in weekly meetings.

Progress toward these objectives will be reviewed and evaluated using the following procedures or methods

· We will be using a weekly log to track the client’s progress and improvements.

· We will be using (PANAS) which is the positive and negative affect schedule that is used to measure mood and emotions.

· Reinforcement of coping strategies to prevent triggers.

· Will encourage journal writing to identify and monitor emotions and triggers.

This assignment is an opportunity to think critically about selecting your practice framework when working with clients. In your work with the client, you will practice from the strengths and ecosystems perspectives and base your intervention in person-centered theory.

You will use the action plan that you created for your client as the basis for this assignment. The goal is to determine what practice model and culturally responsive techniques you will utilize to help your client most effectively reach their goals, and how you will evaluate progress towards the goal. Think about where the client is in terms of their readiness for change and what kinds of conversations you would need to have with them to intervene most effectively in their change process.

Select one of the identified goals from your action plan and build your intervention and evaluation plan around the work you will do with your client to help them meet that goal.

Content requirements:

· Introduction: Discuss your client’s presenting problem and the goal that you have chosen to focus on for this intervention and evaluation plan.

· Discuss why the Task-Centered Model is most appropriate for a generalist practitioner who is working with your client. You will use the textbook and research to discuss why this model is appropriate (and would be helpful) for use with your client.

Task-Centered Model: The task-centered (TC) model is a short-term, problem-solving approach to social work practice. TC helps clients to solve their problems as they define them. Changes in problems are secured by developing and implementing tasks. Respect for the client’s right to be self-determining is emphasized in TC.

The four main steps of task-centered treatment are:

1. Identify the problem

2. Set collaborative goals

3. Create objectives in the form of tasks

4. Evaluate

· Support your decision with two articles from peer-reviewed journals. The articles must be related to social work practice and as closely related to your client’s demographics or circumstances as possible. Articles can be found through the FIU Library and/or Google Scholar. The articles should back up your choice of model.

· Discuss how you would use psychoeducation with your client as part of your practice framework. The psychoeducation model is not meant to be standalone, but rather complements the work done in other practice models. How would you use psychoeducation to support your crisis intervention or task-centered work with your client? What tools, techniques, and/or activities would you use? Be specific.

· Select and discuss three techniques that you will use with your client during the intervention that will help them move along in their planned change process.

Discuss why you selected the technique.

Discuss, in detail, what changes/tasks are on their action plan and how you will use the techniques you selected when talking to your client about the changes/tasks identified.

Example: Using role reversal and/or role play to practice a job interview with a client to help them understand and practice what that encounter will be like with the hopes of helping them decrease their anxiety about the encounter.

· Discuss how you will evaluate if the intervention plan is effectively helping your client meet their goals as identified in the action plan.

The evaluation should include specific outcomes (increase in positive behaviors/feelings, completion of tasks, etc.) and ways that you will utilize evaluation tools such as scales, frequency counts, documentation of completion of tasks, etc.

You must also discuss how frequently you will collect evaluative information (weekly, monthly, every three months, etc.). The evaluation should not be based solely on client self-report or social worker observation.
